If your Pharmacist license has lapsed/expired, complete the Pharmacist License Reinstatement.

If your Pharmacy Technician Registration and/or State Certified Pharmacy Technician Registration has lapsed/expired within the last year, you must complete the Lapsed Pharmacy Technician Registration. Click here to obtain the application.

In-State Renewal Applications

In State (Resident) Renewal Applications

All resident facilities, other than the four facilities listed below, will renew through the Board's Document Submission process. This means that the renewal application must be printed, completed and then uploaded through the Board of Pharmacy website.

Once your renewal application is uploaded you will then be given the option to pay the renewal fee with a debit/credit card or check.

The other available renewal option is submission via mail utilizing the paper application. Utilizing this option requires the renewal application to be mailed into the Board of Pharmacy. Renewal applications that require a payment are not permitted to be sent via email.

To avoid the $50 late fee all facility renewals should be submitted on or before May 31, 2025. Mailed applications must be post marked on or before May 31, 2025 to avoid this late fee.

To prevent a lapse in licensure please submit the renewal application(s) before June 30, 2025. Mailed applications must be post marked on or before June 30, 2025 to avoid a lapsed license. As of July 1st, lapsed licenses will incur an additional $10 per day penalty.

• EMS for Profit Only
• Medical Gas/Legend Device Permit
• Federally Qualified Health Center (FQHC) Permit
• 503B Outsourcing Permit
• Health System Non-Dispensing Drug Outlet Permit
• Central Fill Pharmacy Permit
• Narcotic Treatment Program Permit
• Narcotic Treatment Program Satellite Permit
• Non-Dispensing Drug Outlet Permit (not for EMS Non-Dispensing)

If you are an EMS non-profit, you will not be able to use Document Submission. Print and complete the application. Then email the renewal application to contact.pharmacy@llr.sc.gov.

2025-2026 EMS Non-Profit Non-Dispensing Permit Renewal

Only the below permits are available for online renewal:

• Pharmacy Permit
• Manufacturer/Repackager Permit
• Wholesale Distributor Permit
• Third Party Logistics Permit

Non-Resident Renewal Applications

Out of State (Non-Resident) Renewal Instructions

All non-resident facilities, other than Non-Resident Pharmacy permits (which are opening soon), can upload the completed renewal application along with the required documents and pay the annual renewal fee through the Document Submission link. After you upload your renewal application and any necessary supporting documentation, you will have the option to pay the renewal fee using a debit/credit card or check.

The other available renewal option is submission via mail utilizing the paper application. Utilizing this option requires the renewal application to be mailed into the Board of Pharmacy.

To avoid the $50 late fee all facility renewals should be submitted on or before May 31, 2025. Mailed applications must be post marked on or before May 31, 205 to avoid this late fee.
