Pharmacist

If your Pharmacist license has lapsed/expired, complete the Pharmacist License Reinstatement.

Pharmacy Technician

If your Pharmacy Technician Registration and/or State Certified Pharmacy Technician Registration has lapsed/expired within the last year, you must complete the Lapsed Pharmacy Technician Registration. Click here to obtain the application.

If your Pharmacy Technician Registration and/or State Certified Pharmacy Technician Registration is lapsed/expired 2 years or more, you must complete the Pharmacy Technician Registration Reinstatement application. Click here to obtain the application.

To renew your Pharmacy Technician Registration, click on "Log In Portal" to begin the online renewal.

All registrations must be renewed on or before June 30, 2026 to avoid a late fee of $10 as of July 1, 2026.
