Licensure
Applicants for licensure, registration or certification must complete an application and submit it along with the applicable fee and required documents to the Board office. Applications are valid for one (1) year from the date of receipt. If all required information is not received within this one (1) year period, the application, application fee and required documents must be resubmitted. This includes, but is not limited to, the examination scores, transcripts, license verifications, etc.
